I accept self-pay and BCBS, Aetna, United Healthcare, Oxford, and Oscar insurances. I can provide a “superbill” for you to submit to your insurance for partial/full reimbursement using your out-of-network benefits.

Full payment is required at the time of service. Payment can be made in the following forms:

  • All major credit cards

  • HSA & FSA accounts

  • Bishop/Clergy Pay

Superbill

I am an out-of-network provider. I can provide a “superbill” for you to submit to your insurance for partial/full reimbursement using your out-of-network benefits.

  • A superbill is a detailed receipt outlining the services you received.

  • For many, you can simply submit the superbill to your insurance for reimbursement, making the process straightforward and client-friendly.

Cancellations

As a one time courtesy, $75 will be charged for the first missed appointment that is not canceled 24 hours prior to the scheduled appointment.

Subsequent no shows or late cancelations will be charged $125.
